We at Petstruly.com understand how costly prescription medications for pets are. We are doing all that we can to deliver medications to your home in the most cost-efficient way. At the same time, we strictly adhere to federal and state regulations regarding prescription medications.
Unfortunately, we are unable to accept prescriptions faxed by customers. They must be either faxed from your veterinarian's office or the originals must be mailed to our pharmacy.
There have been cases when pet owners had trouble obtaining a written prescription. Please keep in mind that, while a veterinarian is not required to correspond with the pet owner's supplier, veterinarians are ethically bound to issue a written prescription upon request.
We are also unable to contact your veterinarian regarding your prescriptions. So we ask you to please read the instructions below and help us help you!
All orders for prescription medications require a veterinarian's original prescription.
Sending in only the prescription without an order does not constitute an order. Please place your order before sending in your prescription. Then include your order number when sending the prescription.
Print and hand attached form to your veterinarian for their convenience. Your veterinary hospital can then fax the form to Clark's Greenfield Pharmacy.
New York veterinarians cannot use this form. Instead, the New York Official Prescription Form must be submitted.
Louisiana Residents: We are expanding our licensing permits for Rx items. Until this is complete, we are unable to offer Rx items to Louisiana residents.
Please add the following information to the written prescription if it is missing:
Pet owner's name,
Pet's breed, sex,
Veterinarian's name and phone number.
Additional helpful information, such as:
Order number if it's known,
Customer's name placing the order if it's different from the name on the prescription.
There are two ways to submit a prescription:

To expedite processing, your veterinarian can use the following form to provide additional information: PDF (Acrobat) or Plain Text (HTML) and send it along with your prescription.
If you are sending your original prescription via postal mail, you can help to speed up processing of your order by also faxing your prescription to Greenfield Pharmacy (888) 557-9629 before mailing the original. Please include your order on both.
Louisiana Residents: we are currently unable to process prescriptions from Louisiana
veterinarians nor prescription medications being shipped to Louisiana. We are working
hard to remedy this situation.
Prescription items are NON-RETURNABLE and NON-REFUNDABLE.
